Imperial Animal Hospital Pet Lodge group play liability waiver
I,_______________hereby certify that my dog, _____________________is in good health and has not been ill with any communicable condition in the last 14 days. I further certify that my dog has not harmed or shown any aggressive or threatening behavior towards any person or any other dog. I have read and understand the following:
I understand and I agree that I will not hold Imperial Animal Hospital Inc. or any of its employees or owners liable for any problems that may develop while attending Imperial Animal Hospital Inc. group play provided proper care and precautions are taken. Accept
I understand and acknowledge that Imperial Animal Hospital Inc. is relying on information I provided stating that my dog is in good health and has not harmed or shown any aggressive or threatening behavior towards people or dogs. Accept
I further understand that any dog’s ability to participate in group play is at the sole discretion of Imperial Animal Hospital. Any dog may be separated from other dogs or asked to leave group play if that dog does not meet Imperial Animal Hospital's health and temperament requirements. Accept
I understand and acknowledge that I am solely responsible for any harm caused by my dog while he or she is at Imperial Animal Hospital. I hereby release Imperial Animal Hospital Inc. of any liability arising from my dog’s attendance in group play. Accept
I further understand and agree that there is a risk associated with my dog playing with other dogs such as minor cuts, scratches and paw injuries. I understand that Imperial Animal Hospital staff will do everything they can to prevent injuries, but that not all injuries can be prevented for dogs participating in group play. I am willing to accept this risk and allow my dog to participate in group play. Accept
I understand that not all injuries are considered emergencies and that staff may feel it is okay to notify me upon pick-up of any injuries sustained while participating in group play. If an injury is determined to be serious I will be notified immediately. Accept
I agree to assume all liabilities and responsibilities, financial and otherwise, for the behavior and health of my dog. I further understand and agree that in the event of a medical emergency Imperial Animal Hospital veterinarians will be contacted first for initial treatment, and I will assume full financial responsibility for any such treatment. Accept
